Providence Health & Services Home Health Aide/CNA in Spokane Valley, Washington
Providence is calling a Home Health Aide (CNA) (Day Shift, 1.0 FTE) to Providence VNA Home Health in Spokane, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Prioritizes patient’s needs and meet care plan objectives.
Reports and documents patient status and the care (or service) provided in relation to the individual care plan.
Maintains accurate schedule for patient care.
Arrive at patient home in a timely manner. Communicates problems with schedule to Home Health Manager/Supervisor as soon as possible.
Communicates with patients (and patients’ families), with primary clinician and team members; reporting accurately, and in a timely manner, changes in patient status.•Assigned tasks to include (but may not be limited to):
Monitoring and recording vital signs to include blood pressure,
Changing of non-sterile dressings,
Removal and application of ostomy appliances,
Urine testing according to care plan,
Application of hot or cold packs,
Assisting patients with medications per agency policies.
Records fluid and nutritional intake per agency policy.
Communicates changes in fluid and nutritional intake to primary clinician and team members.
Maintains knowledge of DNR orders for patients and communicates changes in patient status accurately and in a timely manner.
Documents any deviations from the plan of care and contacts the primary clinician and/or Home Health Manager/Supervisor.
Supervisory and Joint visits are completed within time guidelines and documented on the plan of care.
Participates in coordination of care by documenting on the Care Coordination Log in each patient’s home
Attends inservice education meetings and staff meetings
Participates in agency committees or task force
Required qualifications for this position include:
High School diploma or General Equivalency Degree (GED)
Coursework/Training completion of a Certified Nursing Assistant Course
1 year of experience as an aid in an acute or rehabilitative facility
Valid Washington State driver’s license and insured, reliable vehicle
Current Washington State Nursing Assistant Certification..
Healthcare Provider BLS/CPR certification required.
Preferred qualifications for position include:
- Experience as a Certified or Registered Nursing Assistant in home health care
