Providence Health & Services Medical Social Worker - Home Health Spokane Vna in Spokane Valley, Washington


Providence is calling a Medical Social Worker (Home Health Spokane VNA , Day Shift, 0.6 FTE) to Home Health VNA in Spokane County, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.

In this position you will:

  • Assesses the psychosocial, spiritual, cultural and relational needs of the patient/family.

  • Conducts assessments for patients and families to determine financial status and eligibility for various community programs and services.

  • Correlate assessment findings with appropriate interventions and follow up.

  • Develop plans of care based upon physician orders and evaluation of patient needs.

  • Mutually establish a Plan of Care with the patient/family and physician.


Required qualifications for this position include:

  • Master's Degree in Social Work from a school accredited by the Council on Social Work Education.

  • Current Washington State registration as an Agency Affiliated Counselor

  • Requires current valid driver’s license for operation of vehicle.

  • Personal vehicle insurance at prudent coverage levels as determined by PVNA management.

  • 1 year social work experience in a health care setting.

Preferred Qualifications for this position include:

  • Credentials as a current Licensed Clinical Social Worker.

  • Certification by the Academy of Certified Social Workers.

  • 2 to 5 years experience in health care setting.
