Providence is calling an Behavioral Health Clinical Navigator (Psych Administration, Day Shift, 1.0 FTE) to Providence Sacred Heart Medical Center in Spokane, Washington. In this position you will:
All Principal Duties and Essential Functions are performed under the established scope to maintain licensure:
Maintains and promotes positive, open and honest interpersonal relationships with internal and external customers.

Obtains consult requests and prioritizes them for the psychiatrist(s) on-going basis.
Manages the phone inquiries from non-physician clinical staff and coordinates relevant information with the psychiatrist.
Rapidly establish therapeutic alliance. Recognize barrier(s) to establishing therapeutic alliance, adopt modifications in evaluation to effectively overcome the barrier(s).
Conduct behavioral health screening and psychosocial assessments, including, but not limited to SUD screening, suicide risk assessment, safety planning, homicidal ideation.
Develop suicide and homicide risk reduction/mitigation plans that reduce or eliminate this risk for patients and the community.
Provide brief intervention using motivational interviewing and other evidence based approaches, education, referral for services, and interventions for substance use disorder to internal and external partners.
Reduce barriers to individuals obtaining SUD treatment, including but not limited to, arranging transportation, providing continued case management, and co-pay assistance for prescriptions.
Interviews primary nursing and charge staff to identify nursing concerns and plan of care problems.
Coordinate, arrange, and schedule appointments, coordinate care on an outpatient case management basis, and conduct follow up phone calls.
Provides targeted consultation to individual staff, leads team conferences as required for complex patients and provides documentation in the patient’s medical record for those with SUD.
Maintains complete and accurate documentation of all coordination activity to facilitate participation by any other team members.
Collaboratively coordinates care with the assigned SW, conducts patient and family education on community resources recommended for the patient's next level of care.
Coordinates the consultation and provides the point of contact between the referring physician, the patient and family, the nursing staff and the community.
Works cooperatively and collaboratively with an intra disciplinary team for problem solving patient specific situations.
Organizes and/or participates for care conferences for determining the behavioral treatment plan and the disposition of complex and challenging patients.
Organizes and participates in education regarding care of the medical patient with behavioral challenges and mental illness/substance use disorder.
Coordinate data entry and monthly reports.
Maintains knowledge of diagnostic and procedural coding elements required for the billing management of the consultation services.
Provides an on-going leadership role in program development by revising and adapting the consultation service to respond to the changing demands from the customer base and patient need.
Identify and screen concurrently for quality indicators and participate in outcome measurement.
Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise
Actively engage in a structured clinical supervision plan to continually increase competency.
Meets all of the Centers for Medicare and Medicaid and state regulatory requirements
Follows all organizational policies
Works to promote and support all elements of a high reliability organizations
Required qualifications for this position include:
Master's Degree in Social Work (MSW) in an education program accredited by the Council on Social Work Education and approved by the secretary based on nationally recognized standards OR
Master's Degree in Mental Health Counseling or Behavioral Sciences from an approved school
Active Licensed Mental Health Counselor Associate, Licensed Marriage and Family Therapist Associate or Licensed Independent Clinical Social Work Associate issued by the Washington State Department of Health required within 90 days of hire
Preferred qualifications for this position include:
2 years Inpatient or outpatient behavioral health experiences in substance use disorder or mental health
2 years experience in case management and discharge planning
