Providence Health & Services Resident Care Manager - LPN in Spokane, Washington

Providence is calling a Resident Care Manager - LPN (Sub Acute Care I, Day Shift, 1.0 FTE) to Providence St. Joseph Care Center in Spokane, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.

In this position you will:

  • Conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of this state and the policies and goals of this facility.

  • Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.

  • Work with the Interdisciplinary Care Plan Team in developing a comprehensive resident assessment and care plan for each resident.

  • Evaluate each resident’s condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.

Qualifications:

Required qualifications for this position include:

  • Nursing Degree from an accredited college or university or be a graduate of an approved Licensed Practical Nurse (LPN) program

  • Current, unencumbered, active license to practice as a LPN

  • 1 year of experience as a supervisor or case manger in a hospital, long-term care facility, or other related health care facility

  • 1 year experience with the RAI process in long term care facility

  • Experience in rehabilitative and restorative nursing practices

Providence St. Joseph Care Center in Spokane, Washington is located on the campus of Providence Sacred Heart Medical Center & Children's Hospital. Our Transitional Care Unit is designed for patients coming out of the hospital who need skilled nursing care, end of life care, or rehabilitation for those recovering from surgery or who have a functional loss from illness, disease, or an accident. Our Long Term Care unit provides skilled nursing in a residential setting, addressing the needs of our residents through creating a sense of community and home and providing a model of care that is resident-centered.
