Providence Health & Services Social Worker - Pediatric Endocrinology in Spokane, Washington
Providence is calling a Social Worker ( Pediatric Endocrinology , Day Shift, Full Time) to Providence Medical Group in Spokane, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
This social worker will support Pediatric Endocrinology, Peds Surgery and Peds Urology. Schedule is day shift, Mon-Fri.
In this position you will:
Work collaboratively with Case Managers, nursing staff, external organizations and community resources to provide needed services and coordination for transitions in the continuum of care.
Meet immediate needs of patients and families for emergency assistance as resources are available.
Apply the social work process in contribution to an interdisciplinary approach to assessment, planning of care, and intervention.
Facilitate discharge activities and coordination in determining discharge disposition, transportation and connection to community resources.
Provide an important informal leadership by promoting team functioning and performs duties in a manner which reflect the mission and core values of Providence Health Care.
Required qualifications for this position include:
Master’s Degree in Social Work (MSW) from a program accredited by the Council on Social Work education
Must be licensed as a Social Worker (“Advanced” or “Independent Clinical”) in the State of Washington (both are LW credentials), or licensed as a Counselor in the State of Washington while completing supervision requirements toward the Social Worker license.
Preferred qualifications for this position include:
- 2 years hospital experience
